What you will do:

• Conduct comprehensive assessments of individuals and families to identify support needs.
• Develop and implement care plans in collaboration with multidisciplinary teams.
• Monitor, review, and evaluate care plans regularly to ensure they meet the needs of service users.
• Provide advice and support to children, parents, and carers on safeguarding matters.
• Maintain accurate records and prepare detailed reports as required.
• Work closely with external agencies and community resources to ensure a holistic approach to safeguarding.
• Engage in reflective practice and attend training sessions to continually improve professional knowledge and skills.
