Position: Social Worker - Team for older people

Job Summary

We are looking to recruit a committed, creative and resilient Social Worker to support our continuous improvement journey, towards best practice. This is evidenced within our recent positive Inspection.

Key Responsibilities
  • Accept referrals for individuals living in the community, care home residents, and those currently in hospital.
  • Work in multi‑disciplinary teams with families, unpaid carers, third sector and health professionals.
  • Assist with assistive technology and support unpaid carers.
  • Provide reablement and direct payments services.
  • Support individuals living with dementia diagnoses.
  • Assist with hospital discharge and hospital avoidance.
  • Safeguard and protect vulnerable adults.
  • Address self‑neglect and perform Mental Capacity and Best Interest decision‑making.
  • Carry out Continuing Healthcare assessments and interventions under the Mental Health Act.
  • Facilitate Court of Protection procedures.
  • Receive supervision from a dedicated supervisor and mentor, participate in peer support groups and team meetings.

Requirements
  • Enhanced DBS check and Social Care Wales registration.
  • Ability to work for Wrexham County Borough Council (no sponsorship).
