• Provide care to clients in accordance with the individualized Plan of Care developed by the supervising nurse.
• Assist clients with transfers, ambulation, and prescribed exercises as instructed by the supervising nurse.
• Prepare modified diets as directed by the care plan.

• Observe and report changes in clients' vital signs, skin condition, or symptoms to the supervising nurse.
• Remind clients to take prescribed medications as instructed.
• Perform light household tasks necessary for a safe and comfortable home environment, such as cleaning and grocery shopping.
• Report any changes in the client s physical status promptly to the supervising nurse or agency.
• Submit required annual compliance documents (e.g., TB questionnaire).
• Complete 12 hours of mandatory annual compliance training as required by the New Jersey Board of Nursing

• Active New Jersey Certified Home Health Aide (CHHA) license (required).
• Must be at least 18 years of age and able to read and write in English; bilingual candidates are encouraged to apply.
• Must have a physical examination meeting New Jersey State Department of Consumer Affairs requirements for home health care workers.
• Demonstrate compassion, reliability, and professionalism.
