About the role
An exciting opportunity has arisen in South for a Senior physiotherapist to join our "Ketts Oak" Community Therapy service. You will work with and support a team of friendly, approachable Physiotherapists, Occupational Therapists and Community Assistant Practitioners. The post will be based at Wymondham Health Centre and requires travelling out to see patients in their own homes or in care homes in the surrounding villages, as well as potential to provide cover and in-reach into our community bedded unit at Ogden Court.
Responsibilities- Deliver high quality physio therapeutic interventions and health care to patients within a variety of community settings, which may include in-reaching on to the in-patient unit as needed, providing proactive, holistic case management and rehabilitation.
- Utilise health coaching skills to actively engage with patients and their families to ensure expectations are appropriately managed.
- Assess and prescribe exercise programmes to patients with frail and elderly, palliative care, and rehabilitation needs, aiming for quality care and independence where possible.
- Work with the integrated team to facilitate early discharge from hospital and to prevent unnecessary admission.
- Collaborate with statutory and non-statutory agencies to provide a seamless integrated service to our service users.
- Provide advice, support and supervision to junior staff, therapy assistants and students.

6. Senior physiotherapist with experience in community, palliative or rehabilitation settings.
