CNA Home Health – Weston County Health Services
As the Certified Nursing Assistant, this role is responsible for providing outstanding patient care and physical and emotional support for patients on a daily basis.
Essential Functions- Promotes the mission, vision, and values of Weston County Health Services (WCHS).
- Answers resident calls promptly and determines the best course of action to support them.
- Provides physical support for residents with daily activities and personal hygiene, including bathing, dressing, getting in and out of bed, toileting, walking, standing, or exercising.
- Turning and repositioning bedridden residents as required.
- Ensuring residents receive appropriate diet by reviewing dietary restrictions, food allergies, and preferences.
- Obtaining a wide range of information regarding resident condition, treatment plans, and suggested activities as needed.
- Measures and records food and liquid intake and urinary and fecal output and reporting changes to medical or nursing staff.
- Records vital signs, including blood pressure, pulse, temperature, and respiration rate as requested.
- Examines residents to detect issues requiring medical care, including open wounds, bruises, or blood in urine.
- Reminds residents to take medication and nutritional supplements as required.
- Notes observations of resident behavior, including complaints or physical symptoms to nurses.
- Coordinates with medical staff and other departments to ensure efficiency.
- Attends and participates in staff meetings and other assigned meetings.
- Adheres to accreditation and compliance standard guidelines.
- Assists in completing required reports, program reviews and preparation.
- Utilizes good communication skills to maintain positive relationships with clients, providers, co‑workers and other sections in the organization and with other facilities in the community.
- Maintains a professional, organized and clean working environment by following organizational policies, guidelines, and safety standards.
- Completes work assignments in a timely manner.
- Assists visitors with directions whenever requested.
- Always introduces self to patients, residents, families, and visitors when appropriate.
- Reports safety hazards as appropriate.
- Replenishes supplies as needed.
- Follows all infection control practices including hand washing techniques, Standard Precautions and Isolation Precautions.
- Maintains set quality standards when performing job duties.
- Has the ability to do work on his/her own with normal supervision.
- Observes safety‑precautions.
- Has the ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
- Has the ability to read and understand equipment operation manual.
- Cooperates with other personnel to achieve departmental objectives, maintain good employee relations, and interdepartmental objectives.
- Enhances professional growth and development through participation in educational programs, trainings, current literature, in‑service meetings and workshops; shows responsibility for own professional practice and ongoing education and learning.
- Performs other duties as assigned.
- Adheres to dress code, appearance is neat and clean.
- Completes annual education requirements.
- Maintains patient confidentiality at all times.
- Reports to work on time and as scheduled.
- Wears identification while on duty.
- Maintains regulatory requirements, including all state, federal, and local regulations.
- Represents WCHS in a positive and professional manner at all times.
- Complies with all WCHS policies and standards regarding ethical business practices.
- Communicates the mission, ethics, and goals of WCHS.
- Participates in performance improvement and continuous quality improvement activities.
- Attends regular staff meetings and in‑services.
