CNA Home Health – Weston County Health Services

As the Certified Nursing Assistant, this role is responsible for providing outstanding patient care and physical and emotional support for patients on a daily basis.

Essential Functions
  • Promotes the mission, vision, and values of Weston County Health Services (WCHS).
  • Answers resident calls promptly and determines the best course of action to support them.
  • Provides physical support for residents with daily activities and personal hygiene, including bathing, dressing, getting in and out of bed, toileting, walking, standing, or exercising.
  • Turning and repositioning bedridden residents as required.
  • Ensuring residents receive appropriate diet by reviewing dietary restrictions, food allergies, and preferences.
  • Obtaining a wide range of information regarding resident condition, treatment plans, and suggested activities as needed.
  • Measures and records food and liquid intake and urinary and fecal output and reporting changes to medical or nursing staff.
  • Records vital signs, including blood pressure, pulse, temperature, and respiration rate as requested.
  • Examines residents to detect issues requiring medical care, including open wounds, bruises, or blood in urine.
  • Reminds residents to take medication and nutritional supplements as required.
  • Notes observations of resident behavior, including complaints or physical symptoms to nurses.
  • Coordinates with medical staff and other departments to ensure efficiency.
  • Attends and participates in staff meetings and other assigned meetings.
  • Adheres to accreditation and compliance standard guidelines.
  • Assists in completing required reports, program reviews and preparation.
  • Utilizes good communication skills to maintain positive relationships with clients, providers, co‑workers and other sections in the organization and with other facilities in the community.
  • Maintains a professional, organized and clean working environment by following organizational policies, guidelines, and safety standards.
  • Completes work assignments in a timely manner.
  • Assists visitors with directions whenever requested.
  • Always introduces self to patients, residents, families, and visitors when appropriate.
  • Reports safety hazards as appropriate.
  • Replenishes supplies as needed.
  • Follows all infection control practices including hand washing techniques, Standard Precautions and Isolation Precautions.
  • Maintains set quality standards when performing job duties.
  • Has the ability to do work on his/her own with normal supervision.
  • Observes safety‑precautions.
  • Has the ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
  • Has the ability to read and understand equipment operation manual.
  • Cooperates with other personnel to achieve departmental objectives, maintain good employee relations, and interdepartmental objectives.
  • Enhances professional growth and development through participation in educational programs, trainings, current literature, in‑service meetings and workshops; shows responsibility for own professional practice and ongoing education and learning.
Non‑Essential Functions
  • Performs other duties as assigned.
