Your Role and ResponsibilitiesIn this vital position, you will:
- Deliver exceptional nursing care to patients with psychiatric and medical needs in their homes.
- Conduct comprehensive psychiatric and medical assessments, providing recommendations to the patient's medical provider.
- Collaborate with patients, families, and interdisciplinary teams to create and implement effective care plans.
- Oversee LPNs, CNAs, and aides in delivering patient care.
- Educate patients and their families on care plans, medications, and home safety.
- Ensure goals are met and coordinate discharge from services.
To thrive in this role, you should have:
- An active, unrestricted RN license in the State of Michigan (BSN not required).
- A minimum of one year of clinical experience as a Registered Nurse.
- At least one year of psychiatric experience.
- Current CPR Certification.
- Reliable transportation, a valid state driver's license, and current car insurance.
