In this vital position, you will:

• Deliver exceptional nursing care to patients with psychiatric and medical needs in their homes.
• Conduct comprehensive psychiatric and medical assessments, providing recommendations to the patient's medical provider.
• Collaborate with patients, families, and interdisciplinary teams to create and implement effective care plans.
• Oversee LPNs, CNAs, and aides in delivering patient care.
• Educate patients and their families on care plans, medications, and home safety.
• Ensure goals are met and coordinate discharge from services.

To thrive in this role, you should have:

• An active, unrestricted RN license in the State of Michigan (BSN not required).
• A minimum of one year of clinical experience as a Registered Nurse.
• At least one year of psychiatric experience.
• Current CPR Certification.
• Reliable transportation, a valid state driver's license, and current car insurance.
