Psychiatric RN Case Manager

In this vital position, you will:

• Deliver exceptional nursing care to patients with both psychiatric and medical needs.
• Conduct comprehensive psychiatric and medical assessments, providing recommendations to the patient's medical provider.
• Collaborate closely with patients, families, and interdisciplinary teams to develop and implement effective care plans.

As a Home Health Nurse, you will:

• Provide home-based care to patients recovering from illnesses, injuries, and chronic diseases.
• Collaborate with a team of physicians and oversee LPNs, CNAs, and aides in delivering patient care.
• Conduct patient assessments, document progress, and communicate changes to physicians.
• Educate patients and their families on care plans, medications, and home safety.
• Ensure goals are met and coordinate discharge from services.

To thrive in this role, you should have:

• An active, unrestricted Registered Nurse (RN) license in the State of Michigan (BSN not required).
• A minimum of one (1) year of clinical experience as a Registered Nurse.
• At least one (1) year of psychiatric experience.
• Current CPR Certification.
• Reliable transportation with a valid state driver's license and current car insurance.
