Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Wyomissing Health and Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care, working closely with our interdisciplinary team to ensure that our residents receive the best possible care.

Licensed Practical Nurse (LPN) Responsibilities
  • Provide direct patient care, including vital sign monitoring, medication administration, and wound care
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Conduct regular assessments and evaluations to identify changes in patient status
  • Communicate effectively with patients, families, and healthcare team members
  • Participate in quality improvement initiatives and maintain accurate records
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive care
Licensed Practical Nurse (LPN) Requirements
  • Current LPN licensure in the state of Pennsylvania
  • Prior experience (new graduates welcome to apply)
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
