Job Description

Quality Clinical Outcomes / Education Coordinator ) – Wythe County Community Hospital.

Under the general supervision of the Director of Clinical Effectiveness, the Quality Clinical Outcomes and Education Coordinator is responsible for providing support to the Director of Clinical Effectiveness in the development, implementation and monitoring of a comprehensive Patient Safety/Clinical Quality Performance Improvement Program, meeting the elements for designation in the National Quality Program, and oversight/management of staff education including Life Talent Center.

The coordinator assists the director in the recording and maintaining all databases for quality data and generating reports as needed to provide information to appropriate sources related quality. The coordinator also assists in continual readiness in regards to regulatory surveys, both in hospital and in outlying facilities. The coordinator plans and develops the hospital education calendar and serves as the Life Talent Center administrator.

Functions
  • Assists with the implementation of activities according to the approved Patient Safety/Clinical Quality (PSCQ) Performance Improvement Program and the National Quality Program.
  • Assists committee and department chairpersons in identifying areas for potential improvement, using databases such as the Safety Incident Management System or Patient Satisfaction Surveys.
  • Assists in the development of objectives and problem‑oriented criteria.
  • Works closely with all departments in the hospital, providing communication and education regarding quality/safety outcomes.
  • Assists the director in data analysis related to safety practices under the PSCQ. Data abstraction may include but is not limited to:
    • Unacceptable abbreviations
    • Medication Reconciliation Issues
    • Unsafe ordering practices
  • Assists the director with quality reviews to identify opportunities for improvement or problem resolution, including but not limited to:
    • Surgical case review
    • Mortality and Morbidity review
    • Other quality reviews as concerns are identified
  • Assists in the communication of appropriate information from data sources to other departments/committees; thereby coordinating the efforts of various committees, departments and project teams.
  • Collaboratives – Leaders will be assigned based upon the topic of the collaborative. The coordinator will ensure that the collaborative assignments are on track and that the teams meet as assigned by the Health Support Center (HSC).
  • Core Measures – The coordinator will work in coordination with the Core Measures Abstractor in the education of and the supervision of effective participation with core measures as defined by the Centers for Medicare and Medicaid Services (CMS). This education and supervision will require the coordinator to spend time on the units caring for the patients performing chart reviews and education to the staff involved in the care of the patients.
  • Medical Staff/Hospital Committee/Team Meetings – The coordinator will be a member and section team lead of the Continual Readiness Team, ensuring that the tracer activities are completed as scheduled and reported to the leaders of the CRT, the CEO and the team.
    • Collaboratives – as described above.
    • Send meeting notifications to appropriate individuals with updates or revisions.
    • Assist as needed in preparation of materials, graphs, and/or PowerPoint presentations.
    • Active participant in the following meetings:
      Department of Surgery, Department of Medicine, Emergency Department, PSCQ Council, PSCQ Meeting, Performance Innovation and Improvement, Code Blue/Rapid Response Intervention, Continual Readiness, Clinical Informatics, Clinical Documentation Improvement and others as assigned.
    • Assist in evaluation and restructuring of processes identified as inefficient or needing revision as part of the PSCQ Council.
  • Education – Collaborate with nurse managers and staff to assess the learning needs and competencies of staff, ensuring completion of departmental competencies per policy.
    • Serve as Administrator of the Life Talent Center software program and assign continuing education to staff members.
