Per Diem Home Health Nurse Practitioner in Wytheville, VA

Deliver high-quality, patient-centered care as a Nurse Practitioner conducting home health risk assessments across Wythe County. • Flexible shifts available between Monday and Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

• Conduct comprehensive home health risk assessments, including annual evaluations and post-discharge visits
• Review and assess prior and current medical history, existing medical conditions, and medication history
• Provide patient education related to medical conditions and proper medication usage
• Identify gaps in care; offer recommendations to improve patient outcomes
• Administrative tasks related to documentation and visit reporting
• No requirement to develop treatment plans or prescribe medications

• Active Nurse Practitioner License in Virginia
• Board Certification from AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred

• Earn $80-$90 per completed home health evaluation, with an additional mileage stipend and compensation for administrative time
