About the Role:We are seeking a compassionate and skilled LPN to join our nursing team. As an LPN at Alpine Rehabilitation and Nursing Center, you will be responsible for providing direct patient care, including administering medications, performing vital signs, and documenting patient information. You will work closely with our interdisciplinary team to develop and implement individualized care plans, addressing the unique needs of each patient.
Your expertise and empathy will help create a supportive and comfortable environment for our residents, promoting their physical, emotional, and spiritual well-being.
- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Conduct patient assessments, report findings, and implement interventions as necessary
- Collaborate with caregivers to ensure patient needs are met and reported
- Maintains accurate and timely records of patient care, including medications, treatments, and test results
- Participate in ongoing education and training to stay current with industry standards and best practices
- Supports a positive and respectful environment for patients, families, and fellow staff members

- Current LPN licensure in the state of Ohio
- Minimum of 0 years of experience as an LPN
