- $24.77–$30.34 DOE with the ability to go higher for highly experienced candidates

- Monday – Friday 8:00 am – 5:00 pm
- Build program‑specific caseload by seeking out referrals from providers and/or other identified partners to meet program objectives.
- Conduct intake screenings to assess clients, identify immediate needs, and determine appropriate program services.
- Conduct in‑depth client interviews using a systematic social services assessment.
- Refer clients to appropriate agencies/services based on their needs.
- Assist clients with the self‑referral process.
- Meet regularly with clients (often in‑home) to monitor implementation of the service plan, provide advocacy, and resolve problems that interfere with participation.
- Create, update, and maintain each client’s service plan and documentation per program guidelines and funding regulatory requirements.
- Establish and maintain relationships with other service providers/agencies & keep updated on available community services.
- Participate in interdisciplinary team case reviews and other meetings as needed.
- Perform clinical responsibilities in alignment with The Joint Commission (TJC), HRSA, and YVFWC requirements.
- Bachelor’s Degree in Social Work, Behavioral Sciences, Psychology, Child Development, or equivalent OR
- Associate’s Degree with two years’ experience in community social services, public health services, crisis intervention, outreach or linkage programs OR
- High school or GED with four years’ experience in community social services, public health services, crisis intervention, outreach or linkage programs
- Valid driver’s license and proof of automobile liability insurance coverage.
- Bilingual (English/Spanish) preferred; must demonstrate level 10 language proficiency to receive bilingual differential pay.
- Effective verbal, written, and listening communication skills.
- Strong customer relations skills and ability to manage stressful situations.
- Knowledge of medical terminology required.
- Knowledge of cultural differences and their impact on patient care.
- Knowledge of local social and health services.
- Knowledge of how to access local community agencies and resources.
- Ability to develop effective working relationships with social service providers and partners.
- Ability to interact professionally with a variety of clients and employees.
- Basic proficiency with Microsoft Outlook, Word and Excel.
