Licensed Practical Nurse – LPN — Pay Rate Range: $28-$42/HR at Aveanna Healthcare

• Provide licensed practical nursing care in home health settings in compliance with physician orders and established policies and procedures.
• Assess patient health status, document accurately, and communicate findings to the care team.
• Administer medications and treatments as prescribed, monitor patient response, and adjust care plans as needed.
• Collaborate with patients, families, and interdisciplinary team to achieve positive clinical outcomes.

• Must have and maintain an active, unencumbered LPN license
• TB skin test - 2 step (current within last 12 months)
• Six months of prior hands-on nursing experience
• Must have reliable transportation
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
• Continuing Education as required by state
