Key Duties- Conduct thorough nursing assessments and ongoing reassessments
- Create, coordinate, and update individualized care plans
- Provide skilled nursing care in the home setting
- Educate patients and families on care plans, medications, and self-management
- Collaborate with interdisciplinary teams to optimize outcomes
- Current RN licensure
- Strong case management and clinical assessment skills
- Ability to plan, coordinate, and deliver patient-centered care
- Excellent communication and education abilities
- Compassionate, patient-focused approach and ability to work in a team
