How you'll make a difference

As a Transition Facilitator, you will be part of a dedicated team supporting children and young people aged 10 to 25 years. Your work focuses on promoting independence and empowering individuals to achieve their full potential at home, in education, employment and within their wider community. Some of the children and young people you support may have special educational needs and disabilities (SEND).

Based at our Badminton Road office, you will be part of a team that sits within the 0-25 Disabilities Service. They specialise in providing early interventions and supporting key transitions with those referred to the service.

What you will be doing
  • You will assess, monitor, evaluate and review cases and care plans, ensuring they remain focused on achieving identified outcomes and meeting the evolving needs of children and young people.
  • It will be your responsibility to maintain accurate and confidential records, including case notes, care plans and risk assessments, in line with professional and council guidelines.
  • Using a strength-based, person-centred approach, you will provide advice, guidance and practical support to children, young people and their families.
  • Working with families, you will promote awareness of the Local Offer and coordinate support through established networks. including social workers, health professionals, schools, colleges and colleagues in our Education, Health and Care Plan (EHCP) team.
  • You will develop and maintain effective partnerships with a range of external providers and agencies, such as health services, housing organisations and emergency services, ensuring clear and effective communication to achieve positive outcomes.
  • It will be key for you to act in accordance with safeguarding procedures, ensuring children, young people and vulnerable individuals are protected and supported at all times.
What we need from you
  • It is essential that you hold a vocational qualification eg RQF/NVQ or equivalent in Social Care.
  • You will have experience of working with children and young people in a related service area, including those who have complex needs and challenging behaviours.
  • Ideally, you will have a good understanding of relevant legislation, regulations and codes of practice, including safeguarding, the Mental Capacity Act, Deprivation of Liberty Safeguards (DoLS) and the Local Offer.
What you need to know
  • This role is limited term maternity cover, ending 12th October 2027.
  • You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
  • Business insurance will need to be added to your current insurance policy.
  • Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) for both Children and Adults is obtained for this post.
