Case Manager for First Nations Resources

Support underserved First Nations as a Case Manager for Jordan’s Principle with the Dene Nation. Key responsibilities include resource optimization and addressing service gaps.
In this role, you will report to the Director of Health and Social Development, directly impacting First Nations communities lacking their Jordan’s Principle agreement. You'll work alongside families to ensure that resources are effectively utilized and that any service gaps are promptly addressed. Key Responsibilities:

• Assist First Nations without Jordan’s Principle agreements

• Optimize family resource utilization for community support

• Identify and address gaps in available services

Requirements:

• Experience working in First Nations health initiatives

• Strong skills in communication and collaboration
