We are seeking a compassionate and motivated Registered Mental Health Nurse (Band
5) to join our dynamic Community Mental Health Service. This is an exciting opportunity to work within a service that is transforming how mental health support is delivered-challenging traditional boundaries between primary and secondary care, and working collaboratively with Service Users, the Voluntary Sector and Community Enterprise partners.
In this role, you will provide assessment, psychosocial interventions, and evidence based care to a defined caseload of individuals experiencing mental health difficulties. A strengths based, recovery focused approach is central to everything we do.
You'll work in a structured and supportive environment with regular supervision, opportunities to grow skills and confidence, and the chance to contribute to the clinical leadership of the team.
A full driving licence and access to a vehicle with business insurance are essential.
Main duties of the job- Provide high-quality mental health assessments using a recovery focused, strengths based approach.
- Deliver evidence based psychosocial interventions tailored to individual need.
- Build, maintain and appropriately end therapeutic relationships with service users experiencing a range of mental health presentations.
- Work collaboratively with service users, carers, and a wide range of external partners including GPs, voluntary sector organisations, inpatient services, and social care agencies.
- Assess and manage clinical risk, including completing safety plans and contributing to care management processes.
- Recognise changes in risk or clinical presentation and elevate concerns appropriately.
- Participate actively in clinical supervision, team meetings, reflective practice, and ongoing professional development.
- Provide supervision to junior staff as agreed with the Team Manager.
- Maintain accurate, timely clinical records in accordance with Trust policies (e.g., RiO).
- Uphold professional standards and adhere to all Trust policies regarding safeguarding, confidentiality, information governance, clinical governance and health and safety.
- Contribute to service improvement, clinical audit, and the development of innovative ways of working.

Job responsibilitiesAs a Registered Mental Health Nurse within the Community Mental Health Service, you will:
Clinical Practice
- Carry out comprehensive assessments, integrating psychosocial, risk and recovery based frameworks.
- Use therapeutic skills such as motivational interviewing, solution focused practice, and strengths based approaches to support engagement and recovery.
- Develop collaborative Recovery Plans and Risk Management Plans, ensuring these are shared with relevant professionals and agencies.
- Communicate highly complex and sensitive information with clarity and compassion to service users, carers and multidisciplinary colleagues.
- Work autonomously with support and mentorship from a supervising registered practitioner.
Partnership Working & Communication
- Liaise closely with a broad range of internal and external partners including voluntary agencies, primary care, inpatient units, safeguarding teams, housing services and substance misuse services. Build strong relationships with carers and families, using a systemic approach where appropriate.
- Participate in multiagency meetings, professionals' meetings, discharge planning and case conferences.
