The General Practice Support Unit (GPSU) exists to champion, support, and strengthen general practice across Somerset. We are seeking an experienced and strategic Data Transformation Manager to lead the development and delivery of data and analytics capabilities that support primary care transformation, neighbourhood working, and population health management.

This is an exciting opportunity for a motivated individual to work closely with GP practices, Primary Care Networks (PCNs), and wider system partners to drive data-led improvement across primary care services. The role will support the delivery of high-quality analytics, meaningful insight, and improved data quality standards to help practices enhance performance, resilience, and patient outcomes.

The successful candidate will play a key role in shaping reporting and business intelligence solutions, supporting transformation programmes, and promoting the effective use of data across the Somerset healthcare system. They will bring strong analytical and stakeholder engagement skills, alongside experience working with NHS data, reporting tools, and primary care systems.

Main duties of the job

The Data Transformation Manager will lead the development and delivery of data, analytics, and reporting capabilities across the General Practice Support Unit (GPSU), supporting primary care transformation, neighbourhood working, and population health management across Somerset.

The postholder will work closely with GP practices, Primary Care Networks (PCNs), and system partners to develop dashboards, reporting frameworks, and data solutions that provide actionable insight to improve access, resilience, quality, and operational performance. Responsibilities include supporting improvements in data quality and clinical coding, developing automated reporting processes, and helping practices maximise performance and income opportunities through effective use of NHS contractual frameworks including QOF and local enhanced services.

The role will also support the development of programme outcomes frameworks and organisational KPI reporting, ensuring accurate, timely, and meaningful performance data is available to inform decision-making across primary care services.

The successful candidate will provide leadership and expertise in data analytics, support collaborative working across organisations, and promote best practice in information governance, data management, and continuous improvement

1. Neighbourhood & Population Health Strategic Data Leadership

  • Lead the analytics workstream supporting neighbourhood working and integrated models of care.
  • Scope and establish data access pathways required for population health management PHM, working with system partners.
  • Develop and deliver analytics that support proactive care, risk stratification, and service planning.
  • Translate strategic objectives into practical data and reporting solutions for frontline services

2. Access, Resilience and Quality Programme Analytics

  • Lead the development of comprehensive analytics offer for the Access, Resilience and Quality ARQ programme.
  • Design and build dashboards that provide practices with clear, actionable insights into:
  • Access and demand
  • Clinical activity and outcomes
  • Operational performance
  • Enable practices to understand how am I doing? and…
