Position SummaryWorking as part of our Mobile Integrated Healthcare team, the Community Paramedic provides high‑quality, compassionate in‑home and community‑based care to patients who may not require emergency transport but need acute or non‑acute treatment, ongoing or follow‑up medical care, and support. Collaborating with primary care providers, emergency departments, physician practices, hospital systems, social services, and other healthcare teams, the role helps reduce unnecessary emergency‑room visits, improves patient outcomes, supports chronic disease management, preventive care, and post‑hospitalization follow‑up.
Able to provide all functions of an Empress EMS NYS EMT‑Paramedic.

- Shifts Hiring: Full‑time
- Pay Grade: 33.50 +
- Conduct advanced in‑home patient assessments, including physical exams, medication review, safety, and health education.
- Provide urgent / unscheduled patient medical responses.
- Provide Treatment in Place through protocols and physician collaboration.
- Provide support for chronic disease management (e.g., diabetes, heart failure, COPD).
- Follow up with recently discharged hospital patients to ensure recovery and adherence to discharge plans.
- Provide ongoing clinical monitoring and interventions.
- Assist patients with access to social and community services.
- Collaborate with healthcare providers to support and implement patient care plans.
- Maintain accurate and timely documentation of all assessments, interventions, and referrals.
- Monitor patients for changes in condition and escalate care when needed.
- Participate in public health initiatives, health screenings, and community outreach programs.
- Educate and coach patients and families on health maintenance, disease prevention, and self‑care strategies.
- Respond to emergency 911 calls when appropriate to support the EMS system.
- Education: High school diploma or GED required.
- Licensure/Certification:
- Certified Paramedic (NREMT‑P or NYS EMT‑P Certification).
- Completion of a Community Paramedicine program, or Community Paramedic training will be provided for qualified applicants.
- BLS and ACLS, PALS certification required.
- Experience:
- Minimum of 2 years as a field paramedic.
- Experience in community health, primary care, or public health is an asset.
- Strong interpersonal and communication skills.
- Ability to work independently and collaboratively in diverse environments.
- Knowledge of chronic disease management and public health principles.
- Comfort with electronic medical records (EMRs) and mobile health technology.
- Problem‑solving skills and ability to make clinical decisions in non‑traditional environments.
- Cultural sensitivity and ability to work with diverse populations.
- Work is performed in patients’ homes, community centers, and other non‑clinical settings.
- May involve exposure to environmental conditions such as inclement weather, animals, and unsanitary living conditions.
- Requires lifting, bending, and periods of driving or walking.
