In-Home NP​/PA Clinician — Part-Time

Signify Health, part of CVS Health, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-Home Health Evaluations. Visit members in their homes, conduct health assessments and bridge gaps in care using an intuitive iPad workflow.

You will travel locally within licensed states, perform virtual visits as needed, and be eligible for select benefits. Active licenses and board certification are required; multi-state licenses preferred.
