Psychiatric Mental Health Nurse Practitioner; Yonkers

Job Summary

We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric services within post‑acute and residential care environments. This role offers a structured schedule, clinical autonomy, and the opportunity to work with a stable adult and geriatric patient population in a non‑hospital setting.

• Conduct comprehensive psychiatric evaluations and routine follow‑up visits
• Develop, implement, and adjust treatment plans for chronic mental health conditions
• Prescribe and manage psychotropic medications in accordance with best practices
• Collaborate with nursing staff, medical leadership, and interdisciplinary care teams
• Communicate effectively with patients and family members regarding care plans
• Provide patient‑centered, trauma‑informed psychiatric care
• Maintain timely and accurate clinical documentation in compliance with regulatory standards

• Board‑certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Active, unrestricted state license
• Strong interpersonal, communication, and clinical judgment skills
• Ability to work independently while collaborating with care teams

• Experience in long‑term care, skilled nursing, or geriatric psychiatry is a plus
• Prior experience as a Psychiatric RN or Psychiatric NP in a residential or institutional setting is a plus
