Service Care Solutions are recruiting for a Care Navigator to support Primary Care services in the Yorkshire Area. This is a fantastic opportunity to help contribute to providing patients, the right care, at the right time.

Duties:
Supporting patients in accessing appropriate services by signposting to clinical or non-clinical support based on their needs

Managing and triaging patient enquiries (by phone, in person, or online) and directing them to the most appropriate member of the healthcare team

Following safeguarding protocols and escalating any concerns to clinical staff or safeguarding leads as required

Handling sensitive information with discretion and maintaining patient confidentiality at all times
