This is an exciting opportunity to join a forward thinking Practice, providing the highest standard of patient-centred health care.

The successful candidate will be responsible for managing patients with long term conditions as part of a large, experienced clinical team including GPs, ACPs, Practice Nurses, Pharmacists and Health Care Assistants. We will welcome part time applicants to be considered for this opportunity.

We are a supportive training practice with 5 GP partners, Salaried GP, ACP and 2 GP Registrars. We have excellent supporting administrative and clinical teams including Practice Nurses, Healthcare Assistants and Pharmacy team. We have approximately 9,700 patients and operate from purpose built premises. We are a member of the Tolson Care Partnership Network. We use TPP Systm One and are high QOF achievers with a Good CQC rating.

We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations, sexual health services and well-person check-ups.

Qualifications

Essential

• Qualified Nurse.
• Registered with NMC.
• General Practice Nursing experience.
• Cervical screening.
• Long term condition management, ie. Diabetes, Respiratory, Cardio-Vascular.
• Complex wound care.
• Immunisations and vaccinations.
• General Practice Nursing responsibilities.
