Locala Health and Wellbeing is seeking an IT0C Band 6 Occupational Therapist to join the Integrated Transfer of Care (ITOC) Team in Liversedge. The role supports safe hospital discharge, home assessments, and close collaboration with nurses, physiotherapists and social care to maximise independence.
The post is permanent, full-time (37.5 hours) with shifts across 7 days (1 in 4 weekends) and flexible working where possible.
