Job summary

Please note:

This role is 7.5 hours per week, worked across one day, preferably on a Friday.

To be responsible for practice nurse duties and support the HCA. The post holder will ensure nursing services are delivered effectively to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as chronic disease management, health prevention, cervical smears childhood vaccination programme and travel immunisations as well as supporting the practice manager in the reviewing and delivery of clinical policy and procedure.

Main

duties of the job

To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being in conjunction with the practice HCA
To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health, in conjunction with the practice HCA
To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
To diagnose and manage acute and chronic conditions
To prescribe medication for patients in accordance with PGDs and own scope of practice
To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages
To co-ordinate the provision of travel medicine services
To request/ process pathology services & results as necessary
To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
To maintain accurate clinical records in conjunction with extant legislation
To ensure the appropriate maintenance and use of chronic disease registers
To implement vaccination programmes for adults and children
To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
Develop, review and implement nursing protocols in conjunction with the partners

Person Specification Experience Essential

Experience in working in a primary care environment
Experience of chronic disease management
Experience of working autonomously

Experience of working in a GP Practice
Experience of leading a nursing team in a primary care environment
Experience of working as a practice nurse or community nurse
Experience of infection prevention and control measures
