What you will do
  • Conduct holistic Care Act assessments to determine the specific needs of adults with learning disabilities, and contribute to multi-disciplinary evaluation processes.
  • Develop, implement, and review personalised care and support plans that promote autonomy and enhance quality of life.
  • Champion the independence, wellbeing, and informed choice of service users, acting as an advocate where necessary.
  • Address and manage safeguarding concerns, ensuring the protection and welfare of vulnerable adults.
  • Collaborate effectively with families, carers, and a broad range of professionals from health, housing, and voluntary sectors, fostering integrated working.
  • Handle a diverse and complex caseload, incorporating comprehensive risk assessments and crisis intervention strategies.
  • Perform mental capacity assessments and apply principles of the Mental Capacity Act 2005 within your practice.
  • Engage in Best Interests decisions and navigate the complexities of Deprivation of Liberty Safeguards (DoLS) and Liberty Protection Safeguards (LPS) when required.
  • Organise and coordinate care services, placements, and community support, ensuring seamless service delivery.
  • Maintain comprehensive and accurate case records, ensuring all statutory requirements and deadlines are met.
