SIGN is seeking a motivated, empathetic, and youth centered individual, to work with the Team, Program Manager and Case Management Supervisor in so to fulfill the overall objectives of the home. The position will assist in the development and implementation quality programming that will enhance the personal, educational, emotional, and social development of the youth living in the home.

- Must attend the scheduled and varied shifts for days, evenings and weekends
- Provide overall case management for one or more youth
- Comply with the case plans for each resident observing special considerations and instructions
- Assess, evaluate, plan, and revise youth care/support as directed by the AGH Program Manager or AGH Case Management Supervisor
- Protect the health and safety of others by adopting safe work practices
- Collaborate and communicate with coworkers and youth to plan, implement, coordinate, and evaluate youth care
- Help to ensure that the nutritional and health requirements of all residents are met
- Plan, teach and participate in social and recreation opportunities for youth residents
- Efficiently complete required paperwork in written format and with Microsoft Office
- The energy and aptitude to support youth while working well with a team
- Experience working with youth and/or in a residential setting would be ideal
- Post-secondary education and/ or training would be an asset
- A recent (6 months or less), clear Criminal Record Check and Vulnerable Sector Check
- Valid drivers licence
