Meridian Health Care is seeking a Behavioral Health Manager - Housing to support the delivery of high‑quality, patient‑centered services within our Housing program, serving individuals with mental illness, co‑occurring disorders, and histories of homelessness. The manager provides direct care and clinical leadership, mentoring Therapist Assistants, facilitating staff training, and ensuring compliance with housing and clinical documentation standards. This role promotes housing stability, wellness, and community integration through coordinated, trauma‑informed, housing‑first approaches.
Key Responsibilities- Carry a caseload of patients and deliver comprehensive, patient‑centered care to support housing retention, mental health stability, and overall wellness.
- Conduct psychosocial, housing‑retention, and functional needs assessments at intake and ongoing intervals using approved tools and timelines.
- Mentor and guide Therapist Assistants through case consultation, resource support, and ongoing performance feedback.
- Support onboarding and training of new staff through shadowing, modeling, and documentation coaching.
- Facilitate or co‑lead team meetings and interdisciplinary case reviews with clinical, medical, and housing staff.
- Coordinate care with external providers, including behavioral health, primary care, and community‑based services.
- Participate in incident reporting, risk management, and emergency response as needed.
- Promote and uphold trauma‑informed, harm‑reduction, and housing‑first principles in all aspects of service delivery.
- Master’s degree in Social Work, Counseling, or related field.
- Active LISW or LPCC license required.
- Minimum of 3 years of experience in case management, supportive housing, or community mental health.
- Demonstrated experience conducting behavioral health or housing‑related assessments.
- Knowledge of Housing First principles, Medicaid documentation standards, and social determinants of health.
- Experience working with individuals with serious mental illness, substance use disorders, and homelessness.
- Strong written and verbal communication skills, leadership ability, and teamwork orientation.
- Familiarity with local housing systems, tenant rights, and supportive service networks.
- Proficiency with electronic health records (EHR) and Microsoft Office Suite.
- Valid driver’s license and ability to travel locally for home visits and appointments.
