Advanced Heart Failure Cardiology APC Opportunity:

Bon Secours Mercy Health Physicians - Youngstown is seeking a motivated Nurse Practitioner or Physician Assistant to provide outpatient coverage at the CHF clinics zabeth Youngstown Hospital, St. Elizabeth Boardman Hospital, and St. Joseph Warren Hospital. The heart failure program includes 1 nurse practitioner, 3 heart failure nurse coordinators, 20 RNs, a community health worker, and a dietitian who rounds monthly.

This position involves rotating through each clinic to see outpatient post-hospital CHF visits, treat acute HF exacerbations, titrate Guideline Directed Medical Therapy (GDMT), investigate heart failure etiology, assess arrhythmias and cardiac device needs, and address HF risk factors.

Cardiology experience is preferred.
