Homecare with Heart is offering competitive pay ~$30-$31 per hour depending on experience and case type. This position will serve Mahoning County. We are looking for compassionate and experienced nurses to provide home care. As a Home Care Nurse, you will oversee quality and expert care of assigned patients.

• Administer medication in accordance with standards of practice.
• Encourage social and habilitative skills and self‑care independence.
• Perform client treatments in accordance with Plan of Care.
• Assist the consumer in learning appropriate self‑care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document changes, and report any change in client condition to Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to agency standards.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in‑services, orientation and staff meetings.
• Provide service in accordance with agency’s philosophy, standards, policies and scope of practice; participate in performance improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

• Valid State RN License.
• Valid driver’s license (drive required for travel).
• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
• Ability to read and understand agency documents (Plan of Care, flow charts, client charts, policies and procedures).
• Computer proficiency (PC, laptop or tablet).
• Independent thought, problem‑identification, information gathering, and ability to contact a supervisor when needed.
• Excellent customer service and commitment to client care while following agency and program rules.
