LTSS Service Coordinator - Clinician (Case Manager) (LSW, LCSW, LPN)
Hiring statewide across Ohio.
Field‑based role; associates travel to client sites or designated locations as required, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands‑on engagement.

The LTSS Service Coordinator‑Clinician works under the direction/supervision of an RN, with overall responsibility for each member’s case. Within the scope of licensure, the Clinician assists the RN with telephone or face‑to‑face assessments, identification, evaluation, coordination and management of members’ needs—including physical health, behavioral health, social services and long‑term services and supports.
How you will make an impact- Assists the responsible RN in identifying members for high‑risk complications and obtains clinical data as directed.
- Identifies members that would benefit from an alternative level of care or other waiver programs.
- Provides all information collected to the RN for verification, interpretation, and development of care plans that meet members’ needs.
- Participates in coordinating care for members with chronic illnesses, co‑morbidities, and/or disabilities, ensuring cost‑effective and efficient utilization of health benefits.
- Applies current disease processes, terminology and clinical guidelines to make informed decisions that support member care.
- LPN/LVN, LSW, LCSW, LMSW, or other non‑RN license in accordance with applicable state law, plus a nursing diploma or an associate’s degree in nursing or a related field, and at least 2 years of experience working with individuals with chronic illnesses, co‑morbidities, and/or disabilities as a Service Coordinator or similar role (or an equivalent combination of education and experience).
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other non‑RN license as allowed by state law.
- Strong preference for case management experience with older adults or individuals with disabilities.
- BA/BS in Health/Nursing or related field is preferred.
