Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality, expert care of your assigned homecare patients. Pay for LPN: $26-28 per hour depending on experience and availability.
The LPN provides skilled visits in patients' homes.
This position will be serving Mahoning County.
Responsibilities- Administer medication in accordance with standards of practice.
- Encourage social and habilitative skills and encourage self‑care and independence.
- Perform client treatments in accordance with the Plan of Care.
- Assist the consumer in learning appropriate self‑care techniques.
- Prepare clinical and progress notes to document care provided.
- Monitor, document accordingly and report any change in client condition to the Clinical Director/Clinical Supervisor.
- Promote activities to achieve a safe and healthy environment.
- Observe aseptic technique when performing client procedures/care.
- Collaborate with other members of the clinical team to develop and improve the Plan of Care.
- Assist the client in achieving established goals in the Plan of Care developed by the RN.
- Maintain productivity according to the standards of the agency.
- Maintain license, CEUs and mandatory employment requirements.
- Participate in client care meetings, in‑services, orientation and staff meetings.
- Provide service in accordance with the agency’s philosophy, standards, policies and scope of practice. Participate in Performance Improvement and adhere to HIPAA guidelines.
- Other duties as assigned.
- Valid State LPN License.
- Valid driver’s license and use of automobile is required due to the travel involved.
- Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
- Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
- Must be able to use a computer, laptop or tablet.
- Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
- Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served.
