* Homecare with Heart is offering you the most competitive pay scale in the area. We're looking for compassionate and experienced nurses to provide care for our clients in their homes. As a Home Care Nurse, you will be responsible for overseeing the quality and expert care of your assigned home‑care patients. RN: $30 per hour. The RN provides skilled visits in patients' homes.

This position will serve Mahoning County. Homecare With Heart is a locally based provider of home‑based private duty, skilled nursing, personal care, pediatric nursing, physical therapy, as well as education and advocacy, proudly serving the Tri‑County area of Ohio since 2004.

• Administer medication in accordance with standards of practice.
• Encourage social habilitative skills and self‑care and independence.
• Perform client treatments in accordance with Plan of Care.
• Assist the consumer in learning appropriate self‑care techniques.
• Prepare clinical and progress notes to document care provided.
• Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor.
• Promote activities to achieve a safe and healthy environment.
• Observe aseptic technique when performing client procedures/care.
• Collaborate with other members of the clinical team to develop and improve the Plan of Care.
• Assist the client in achieving established goals in the Plan of Care developed by the RN.
• Maintain productivity according to the standards of the agency.
• Maintain license, CEUs and mandatory employment requirements.
• Participate in client care meetings, in‑services, orientation and staff meetings.
• Provide service in accordance with agency’s philosophy, standards, policies and scope of practice; participate in performance improvement and adhere to HIPAA guidelines.
• Other duties as assigned.

• Valid State RN License.
• Valid driver’s license and use of automobile required due to travel involved.
• Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities.
• Ability to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
• Proficiency with computer, laptop or tablet.
• Strong independent thinking, problem recognition, information gathering and the judgment to contact a supervisor when appropriate.
• Commitment to excellent customer service and exceptional client care while staying within the rules of the agency and each program served.
• Oasis experience required.
