Freedom Care is seeking a Case Manager, RN for our Ohio team to conduct in-home assessments, orient new patients and caregivers, and provide ongoing care management and support. The role requires frequent travel to patients' homes in Youngstown, OH.
As part of a collaborative care team, you will assess needs, develop individualized care plans, educate patients and PCAs, and ensure high-quality, compliant services with timely communication to the broader care network.
