We currently have a position available for an experienced Home Health Speech Language Pathologist (SLP) to join our dynamic team!

Responsibilities:
  • Provides speech therapy services to patients according to a written physician's plan of care, including assessment, evaluation, and participation in developing the plan of care. Evaluates home environment.
  • Delivers a full range of Speech-Language Pathology services as ordered by a physician.
  • Enhances or maximizes the patient's communication abilities.
  • Assists in developing and reviewing the plan of care as necessary.
  • Prepares and submits clinical and progress summaries based on goal attainment.
  • Participates in discharge planning for patients.
  • Prepares and submits clinical progress summaries as directed by organizational policy.
  • When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two weeks.

    Requirements:
    • Master’s degree in Speech-Language Pathology from an ASHA-certified program.
    • State license and certification as a Speech-Language Pathologist in Michigan.
    • Minimum of one year healthcare experience.
