- Monitor and document vitals, symptoms, and changes in condition.
- Assist with ADLs such as repositioning, hygiene, mobility, etc.
- Support patient well-being with compassionate and attentive care.
- Collaborate with families, caregivers, and the healthcare team.
- Active RN or LPN license in the state of Michigan.
- Experience in home health, private duty, or long-term care preferred.
- Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required).
- Strong communication skills and a passion for patient-centered care.
