POSITION DESCRIPTION:

Provides professional nursing care for patients following established standards and practices, including triage (telephone and walk-in visits) and care needs between office visits. Demonstrates clinical leadership and oversight within the RN scope of practice.

ESSENTIAL JOB FUNCTIONS:

The following functions may vary among RNs. Some may focus on specific responsibilities such as prior authorizations or indirect support for provider inboxes.

  • Efficiently triages patient needs via telephone and during visits per IHA policies and protocols.
  • Documents clinical findings clearly and concisely according to IHA policy.
  • Uses critical thinking within licensure scope to assess patient needs and involves providers as needed.
  • Enters orders (medications, labs, radiology, referrals) following provider instructions and guidelines.
  • Adheres to clinical and nursing protocols as applicable.
  • Schedules patient appointments as needed.
  • Encourages and supports patient portal usage.
  • Provides scope-appropriate nursing education during patient interactions.
  • Collaborates with providers, care team members, and external resources to ensure quality care.
  • Supports physicians and APPs with indirect work, including test results and order management.
  • Assesses patients for care gaps in prevention and disease management.
  • Maintains accurate medication lists, performs reconciliation, and processes refills per protocol.
  • Works with the care team to co-manage indirect patient work.
  • Coordinates with insurance, DME, home care, and other partners to meet patient needs.
  • Reviews medical history to support documentation and prior authorization processes.
  • Assists patients with social health determinants and resource navigation.
  • Participates in process improvement initiatives.
  • Serves as a resource to staff; may supervise LPNs and Medical Assistants as appropriate.
  • Performs clinical support tasks as outlined in the Clinical Support Staff Scope of Practice.
  • Attends meetings, training, and participates in committees as required.
  • Participates in weekly care meetings and daily huddles as applicable.
  • Performs additional duties as assigned.

      ESSENTIAL QUALIFICATIONS:

      EDUCATION: Graduate from an accredited RN program.

      CREDENTIALS/LICENSURE: Valid RN license in Michigan, CPR certification.

      MINIMUM EXPERIENCE: 1-2 years clinical experience preferred, but not required.

      POSITION REQUIREMENTS (ABILITIES & SKILLS):

      • Accurately obtains vital signs and patient history.
      • Basic knowledge of medical terminology and clinical relevance.
      • Proficient in computer and EMR systems.
      • Excellent communication and interpersonal skills.
      • Team-oriented with a courteous demeanor.
      • Ability to cross-train and adapt to changing priorities.
      • Strong organizational and problem-solving skills.
      • Capable of performing calculations and understanding compliance standards.
      • Handles confidential information appropriately.
      • Willing to travel to other sites as needed.
      • Completes required training within the introductory period.

      MINIMUM PHYSICAL EXPECTATIONS:

      • Prolonged keyboarding, phone work, and charting.
      • Ability to lift over 50 lbs and assist with patient transfers.
      • Visual and manual dexterity required for equipment operation.
      • Good hearing and speaking skills for communication.

      MINIMUM ENVIRONMENTAL EXPECTATIONS:

      Work in a medical setting with exposure to patient care equipment, hazardous materials, and possible contagious diseases. Interaction with distressed or ill patients may be stressful and require prioritization skills.
