The Certified Nursing Assistant (CNA) provides direct patient care and supportive services to residents of Casa Serena Congregate Living, Inc. CNAs perform duties under the supervision of a LVN/ RN and in accordance with physician orders, resident care plans, Title 22 California Code of Regulations, California Department of Public Health (CDPH) requirements, and facility policies and procedures.
Essential Nursing Care Duties- Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and feeding.
- Assist with ambulation, transfers, positioning, and use of assistive devices.
- Measure and record vital signs, intake and output, and weights as assigned.
- Observe, report, and document changes in resident condition promptly.
- Provide comfort, safety, and emotional support to residents.
- Support infection prevention and control practices.
- Provide snacks, nourishment, and fluids when the kitchen is closed using only items consistent with each resident’s physician-ordered and RD-approved diet.
- Ensure adherence to therapeutic diets, texture modifications, fluid restrictions, and allergy precautions.
- Document food and fluid intake as required and report concerns to the licensed nurse.
- Carry out resident activities in accordance with the facility’s weekly activities plan.
- Modify activities as needed to meet individual resident interests, abilities, and care plan goals.
- Encourage participation in individual and group activities to promote socialization, cognitive stimulation, and emotional well-being.
- Protect resident rights, dignity, privacy, and confidentiality at all times.
- Comply with abuse prevention and mandatory reporting requirements.
- Maintain a safe environment and report hazards, incidents, or injuries promptly.
- Participate in emergency preparedness and response activities.
- Document care provided accurately, timely, and objectively per facility policy.
- Record ADLs, vital signs, intake/output, weights, activities participation, and nutritional support as assigned.
- Report and document abnormal findings immediately to the licensed nurse.
- Participate in required facility in-services, mandatory trainings, and competency validations.
- Ensure documentation and practice reflect resident-centered care and compliance with Title 22 requirements.
- Current, active California Certified Nursing Assistant (CNA) certification, in good standing.
- Background clearance as required by law.
- CPR/BLS certification preferred.
- Ability to follow written and verbal instructions.
- Physical ability to perform CNA duties.
- Basic Computer Skills
