Healthy Living at Home - Palm Desert, LLC is in search for a professional and caring LICENSED VOCATIONAL NURSE (LVN) to provide work as a team member and provide direct patient care to our Home Health patients in the greater San Bernadino County - Yucca Valley, High Desert, CA areas.

Position Overview

The Intake Coordinator - Licensed Vocational Nurse (LVN) is responsible for managing all aspects of the patient intake process to include obtaining referral information, physician orders, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Essential Job Functions

• Provides direct patient care as defined in State’s Name Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Basic Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• One (1) year experience as Licensed Practical or Vocational Nursing
• Current Licensed Practical or Vocational Nurse.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication, and organization skills.
• Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
• Current CPR certification.

Preferred Qualifications

• Two (2) years of experience in Licensed Practical or Vocational Nursing
• Bilingual

Physical Requirements

Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary.

The employee must be able to lift and/or move up to 10 lbs. on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
