Job Overview
We are seeking a dedicated and experienced Patient Care Coordinator to join our healthcare team. The ideal candidate will possess a strong background in various medical settings, including critical care, emergency medicine, and hospital medicine. As a Care Coordinator, you will be responsible for coordinating patient care, ensuring effective communication among healthcare providers, and facilitating discharge planning to promote optimal patient outcomes.
Responsibilities- Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
- Collaborate with multidisciplinary teams to coordinate patient services across various settings, including inpatient, outpatient, and post-acute care.
- Monitor patient progress and adjust care plans as necessary based on clinical evaluations and patient feedback.
- Ensure compliance with HIPAA regulations and maintain accurate medical records throughout the care process.
- Provide education to patients and families regarding treatment options, discharge planning, and ongoing care needs.
- Utilize knowledge of medical terminology, physiology, and diagnostic evaluation to support clinical decision-making.
- Engage in utilization management to optimize resource allocation while maintaining high-quality patient service.
- Facilitate communication between patients, families, and healthcare providers to enhance the overall patient experience.
- Experience in ICU, emergency room, hospital medicine, family practice settings is highly desirable.
- Knowledge of geriatrics, pediatrics, behavioral health, infectious disease care, and medical-surgical practices is beneficial.
- Bachelor's Degree Preferred.
- Proficiency in case management principles and discharge planning processes.
- Understanding of managed care systems and outpatient clinic operations.
- Excellent patient assessment skills with the ability to interpret vital signs effectively.
- Familiarity with post-acute care experiences and intake processes is advantageous.
- Ability to work collaboratively within a team-oriented environment while demonstrating strong communication skills.
