Care Manager, Telephonic Nurse

The Care Manager, Telephonic Nurse will assess and evaluate enrollees' needs to achieve or maintain optimal wellness. This role is ideal for experienced nurses who can guide enrollees and families towards appropriate resources for their care.

• Pay: $71,100 - $97,800/year

• Residency: Must reside in Oklahoma

• Assess and evaluate enrollees' needs and requirements.
• Guide enrollees and families towards appropriate resources.
• Employ strategies to manage enrollees' behavioral, physical, environmental, and psycho-social health needs.
• Monitor assessments and evaluations to ensure enrollees are progressing toward desired outcomes.
• Create enrollee care plans.
• Perform telephonic and face-to-face assessments of members' needs.
• Collaborate with providers and community services for quality outcomes.
• Follow established guidelines and procedures.
• Engage with the interdisciplinary care management team.
• Coordinate in-person meetings between care managers and enrollees.

• Active Registered Nurse (RN) license, Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW) required.

• Minimum 1-year clinical experience.
• 2 or more years of care management experience.
• Experience working with Medicaid and/or Medicare enrollees.

• RN (Registered Nurse) - State Licensure required, OR
• LPC (Licensed Professional Counselor) - required, OR
• LCSW (Licensed Clinical Social Worker) - required
