Christelijke Vacature zoekt een Trajectbegeleider in Zaandam voor het coördineren van zorgtrajecten voor vaste bewoners. Je werkt samen met begeleiders om zorgplannen op te stellen en voert ondersteuning uit voor bewoners met justitiële achtergrond.
Met een HBO-diploma in Social Work en de bereidheid om een Verklaring Omtrent Gedrag te overleggen, ben je welkom om te solliciteren. Een contract voor 32-36 uur per week wordt aangeboden met voordelen zoals pensioen en reiskostenvergoeding.
