LPN Pediatric Private Duty

Overview

We are hiring a Pediatric Private Duty LPN (Licensed Practical Nurse) in the Zachary area to provide pediatric private duty care. We offer full‑time and part‑time hours, flexible schedules, and a culture of caring, belonging, and trust.

• Provide direct 1:1 patient care in patients’ homes.
• Document clinical visits thoroughly in the electronic medical record and use the agency’s electronic visit verification program.
• Participate in care coordination, reporting patient needs and changes in condition, and attending patient case conferences.
• Communicate effectively with patients, families, physicians, clinicians, agency management staff, case coordinators, and community case managers.
• Maintain confidentiality of patient health information per agency policy.
• Follow all applicable infection control procedures and act in compliance with the State LPN/LVN Practice Act.

• Current LPN licensure in the state of practice.
• Current CPR certification.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred previous home care experience.

• Must have current LPN licensure in the state of practice.

• LA:
  Current LPN licensure in LA without restrictions. At a minimum, one year of experience working as an LPN.
