Summit Home Care & Hospice is searching for an experienced and motivated Registered Nurse (RN) to join our Home Health team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

Compensation

Base pay: $70,000.00/yr – $80,000.00/yr (Overtime Eligible)

Work Hours & Location
  • Shift: Dayshift, Monday - Friday (40 Hours/Week)
  • Location:

    Zanesville, OH & surrounding areas
  • Expected Travel: 30 mile radius (Mileage Reimbursement, $0.48/Mile)
Responsibilities
  • Plan, organize and direct home care services with emphasis on community health education.
  • Build from community resources to plan and direct services to meet needs of individuals and families.
Patient Care
  • Complete initial assessment of patient and family to determine home care needs.
  • Provide a complete physical assessment and history of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs.
  • Initiate plan of care and make necessary revisions as patient status changes.
  • Use health assessment data to determine nursing diagnosis.
  • Develop care plan establishing goals based on diagnosis and incorporate therapeutic, preventive, and rehabilitative nursing actions. Include patient and family in planning.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Provide direct patient care as defined by State Nurse Practice Act.
  • Counsel patient and family in meeting nursing and related needs.
  • Provide health care instructions as appropriate per assessment and plan of care.
  • Identify discharge planning needs and implement prior to discharge.
  • Act as Case Manager when assigned by Clinic Manager and coordinate patient care for assigned caseload.
Communication
  • Prepare clinical notes and update primary physician at least every 60 days.
  • Communicate with physician regarding patient needs and report changes; obtain orders as required.
  • Communicate with community health professionals to coordinate the care plan.
Additional Duties
  • Participate in on-call duties as defined by the on-call policy.
  • Ensure arrangements for equipment and necessary items/services are available.
  • Instruct, supervise, and evaluate home health aide care provided every two weeks.
Qualifications
  • Graduate of an accredited school of nursing, associate or bachelor’s degree.
  • Valid/active nursing license.
  • Licensed driver with a personal automobile in good working order and insured per company requirements.
  • Valid CPR certification.
  • Recent TB test.
  • Prior home health or acute care experience preferred.
  • Management experience preferred but not required.
  • Excellent observation, verbal and written communication, problem solving, basic math skills; nursing skills per competency checklist.
  • Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending, kneeling or crouching.
  • Must be able to lift, position or transfer patients and lift supplies and equipment.
  • Must have visual acuity and hearing to perform required nursing skills.
