You are a professional caregiver who provides assessment, counseling, treatment, referrals, and advocacy services for patients. • Participates in interviewing persons served and assessing them through mental status evaluation, and reviews records from other care staff.
• Selects and completes appropriate therapeutic interventions based on clinical knowledge (individual, group, family, experiential, and evidence-based models).
• Collaborates with treatment team members regarding patients’ progress through interdisciplinary meetings and record reviews.
• Ensures medical record documentation complies with protocols; progress notes are entered timely and according to procedures.
• Coordinates services and community resources as needed and assists persons served in obtaining them.

Education/Experience:

• Current state licensure in an accredited Master’s level program (Social Work, Psychology, or Professional Counseling).
• Obtain and maintain certification commensurate with provided treatment.
• Trained to work with the patient age group served; annual age-specific education and demonstrated competence.
• One to two years of postgraduate clinical experience.
