Een zorgorganisatie in Zeist zoekt een Casemanager Dementie voor 24-28 uur per week tijdens zwangerschapsvervanging. Je begeleidt en adviseert cliënten met dementie zodat zij de zorg krijgen die ze nodig hebben. Vereisten zijn een hbo-verpleegkunde of hbo Social Work diploma, enkele jaren ervaring met dementiezorg en uiterlijke voorzieningen zoals een auto. Het salaris is volgens de CAO VVT, en je ontvangt extra's zoals een 13e maand en vakantiedagen.
