Qua Rijn zoekt een HBO-verpleegkundige voor het wijkteam in Bunnik, Odijk en Werkhoven. Je hebt vaste werkdagen en coördineert zorg aan cliënten thuis, stemt af met huisarts en familie en houdt vaart in het zorgproces.
Je werkt nauw samen met een teammanager en het verpleegkundig team; je brengt zorg en planning in kaart, voert indicaties uit en bewaakt afspraken zodat zorg daadwerkelijk thuis werkt.
