De JP van den Bent stichting zoekt een coördinerend begeleider in Zwolle. In deze rol ben je verantwoordelijk voor het maken, uitvoeren en evalueren van ondersteuningsplannen. Je werkt samen met collega's en cliënten om de best mogelijke zorg en begeleiding te bieden.
Wij verwachten dat je een relevante mbo 4- of hbo-opleiding hebt afgerond en ervaring hebt met coaching of zorgcoördinatie. Wij bieden een tijdelijke arbeidsovereenkomst met uitzicht op een vast dienstverband.
