Role Overview:
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely and chronically ill patients in their homes. They document clinical notes, coordinate patient care activities, and collaborate with the healthcare team to deliver high-quality care, following state laws and infection control procedures. Supervised by a Registered Nurse or designated supervisor, the LPN participates in care coordination, reports patient needs, and communicates effectively with patients, families, and clinicians.
Responsibilities:- Provide clinical services within scope of practice and plan of care
- Thorough documentation of patient visits in electronic records
- Participate in care coordination and communication with the care team
- Current unrestricted LPN licensure and CPR certification
- Valid driver’s license, dependable vehicle or transportation access
- Ability to work in diverse home environments
- Preferred: home care experience, multitasking, independence, strong communication and organizational skills
Compensation ranges from $20.00 to $35.72 per hour, with benefits including health coverage, incentives, 401k, and stock options. The role emphasizes equitable, community-focused care and values diversity and inclusion.
